Intake Form
The TensorCase Intake Form integrations allows users to create a public form that can be submitted to create a new case, e.g., for use cases like employee complaint forms or whistleblower forms.
Why is our Intake Form special?
When users submit into our Intake Forms, we use AI to create and fill out a new Case for you!
How this works:
- Configure the questions you want to ask responders (we provide a template to start).
- Publish your Intake Form.
- When a responder submits, their response automatically gets populated as a new Case in the Cases page via AI!
Creating a new Intake Form
In order to create a new Intake Form:
- Visit the Integations page by clicking
Integrations
on the left side navigation. - Once on the
Integations
page, click+ Integration
button. - In the popup, select
Intake Form
.

By default, we have an example whistleblower form with sample questions populated.
If you don’t want to use some of our questions, please delete them and add your own.
Editing an Intake Form
Visit the Intake Form you would like to edit on the Integrations
page.
The Intake Form consists of multiple Steps. You can think of a Step as a statement presenting information (text) or an input collecting information from the responder.
You can edit the Intake Form by creating, modifying, or deleting different Steps.
Creating a New Step
In order to create a new Intake Form Step, click +
button below an existing Step or the + Add Step
button at the bottom of the page.
Editing an Existing Step
You can modify an existing Step in the following ways:
Change the Type
of the Step
Currently, a Step can be of type:
Informational
: Just text to present to the responder
Text Answer
: Text input to collect from the responder
Date/Time
: Date and time input to collect from the responder
Multiple Choice
: Choice based input to collect from the responder

Click the existing type to change the type for the Step.
Change Step Text or Placeholder
Click on the Step text to change the informational statements, questions, or input placeholders.
Multiple Choice
For Steps of type Multiple Choice
, you can configure the following options:
- Add, edit, or remove potential choices
- Max Selections:
Single
to only accept 1 choice orMultiple
to accept multiple choices

Display Condition
For each Step, you can set a display condition, which determines whether the Step will appear based on specific filter criteria. Display conditions are currently supported on Steps of type Multiple Choice
.
To set a display condition:
- Click the
+
button next toDisplay If
. - Select the Step you want to base the condition on.
- Choose the options that need to be selected for the condition to be true.
Key points:
- If you select multiple Steps as
Display If
conditions, all conditions must be true for the Step to appear. - If you select multiple options within a single
Display If
Step condition, at least one of the options must match true for the Step to appear.

For example, if the above display condition is present on a Step, then that Step only displays if
Step 5 has multiple choice Choice A
.
Rearrange Steps
In order to rearrange the ordering of Steps, drag and drop Steps using the left side of the Step block (a grab cursor should appear).
Deleting a Step
In order to delete a Step, click the trash delete icon Remove Step
button.
Form Options
Various form options are present at the top of part of the page.
Form Preview
In order to preview a form, click the Preview
icon button at the top right of the page. This will show the current
form in a preview mode (not yet published).
Form Publish
In order to publish a form, click the Publish
button at the top right of the page. This will publish the form as public link.
In order to view the public link, click the Open responder link
icon button at the top right of the page.
If you want to copy the public link to your clipboard, click the Copy responder link
icon button at the top right of the page.
Unpublish a Form
In order to remove a public form, click the ...
options next to the Intake Form name and then select Unpublish
in the menu.
This will unpublish the public link.
Using a Public Form
Once a form is published and public, responders can fill it out and submit. Once submitted, our system will create a new Case for the submission and use AI to fill out the Case Details tab for the new Case! That’s what makes our Intake Form so special - no configuration or mapping needed!
Users that take access to newly submitted Cases will also receive notifications from notifications@tensorcase.com. Please make sure to whitelist this email.
Deleting a Form
In order to delete an Intake Form integration altogeter, click the ...
options next to the Intake Form name and then select Delete
in the menu. Follow the confirmation popup.
